Davenport, IA — Providing effective treatment for carpal tunnel syndrome (CTS) is of primary concern to Dr. Tyson Cobb, an orthopaedic hand surgeon in the Quad Cities. To better assist patients suffering from this common condition, he helped to develop a procedure called ultrasound-guided micro-invasive carpal tunnel release, which utilizes the innovative Sonex SX-One MicroKnife™. Part of what makes this procedure so unique, Dr. Cobb reveals, is the use of ultrasound technology to guide the process and identify essential anatomic structures. Once the area of concern is identified, he can insert the SX-One MicroKnife™ through a tiny incision and reduce pressure on the median nerve—the cause of carpal tunnel syndrome.

Dr. Tyson Cobb says the SX-One MicroKnife™ has several built-in safety features, including a blade that extends and retracts, as well as inflatable bumpers. Both elements are designed to minimize risk of harm to surrounding tissues in the treatment area. The diminutive size of the device also means the incision can be as small as four or five millimeters, or about the size of the top of a pencil eraser. Dr. Cobb adds that sutures typically will not be needed to close the incision; an adhesive bandage is sufficient.

As the first surgeon in the state of Iowa to perform the treatment, Dr. Cobb hopes to bring relief to many individuals suffering from CTS who have been unable to find a suitable solution. Types of Biceps Injuries and Treatment Options

Whether performed as part of demanding exercise regimen or as an occupational duty, heavy lifting can place great strain on the biceps muscles and tendons. When overstressed, these tissues can be damaged, typically leading to significant pain and debilitation. There are three main types of biceps injuries: tendinitis, tendon dislocation, and tendon tear or rupture.
Tendinitis usually occurs as a result of overuse and often can be healed with rest and time, particularly if the injury is minor. As with many biceps injuries, the discomfort may originate from the shoulder or the elbow. During your initial consultation, Dr. Cobb will determine the nature of your injury.
Tendon dislocation, sometimes called tendon instability or tendon subluxation, generally occurs as a result of complications from a shoulder injury, such as a rotator cuff tear, where the long head of the biceps tendon migrates out of the anatomical groove where it should sit. Generally the two conditions will need to be treated together.
Biceps tendon tear or rupture can occur as a result of overuse or traumatic injury. Tears can be partial or complete and can occur at the shoulder (long head) or at the elbow. Minor partial tears often can be addressed with non-surgical treatment. A full tear, also called a rupture, typically results in shortening of the muscle, creating a bulge. Biceps tendon tears of the long head can cause a “popeye” muscle in the arm. Tears of the biceps at the elbow result in weakness and should be surgically repaired to restore strength in active people.

OS Sets the Standard in Patient Care Internationally

The staff at Orthopaedic Specialists (OS) was recently awarded scholarship money to present original research at the 2015 American Association for Hand Surgery (AAHS) Annual Meeting in Nassau, Bahamas. This meeting was held in collaboration with British Society for Surgery of the Hand (BSSH). Although Dr. Tyson Cobb is already well known worldwide for his minimally invasive surgical innovations and research, ‘Team Cobb’ has ‘stepped up’ and is setting a new standard for patient care. OS is proud to announce that due to the collaborative efforts of their staff at the Center of Excellence for Hand and Upper Extremity, Team Cobb was able to present the results of three research studies at the international meeting.
Two of the research articles informed other surgeons and therapists of the minimally invasive surgical techniques developed by Dr. Cobb and treatment outcomes for patients with thumb arthritis and chronic distal biceps tendon ruptures (the biceps muscle tears where it attaches to the front of the elbow). Patients who underwent Arthroscopic Resection Arthroplasty for thumb arthritis were followed for a minimum of five years. These patients were found to have good results with respect to pain, strength, function, and satisfaction. Although chronic biceps tendon rupture (BTR) is often complicated by severe retraction and contraction and therefore traditionally thought to require tendon grafting, Dr. Cobb performs the surgery endoscopically, through a small incision without the need for the graft. After following their patients who underwent this procedure for more than seven years, this technique was found to be a safe and effective solution for symptomatic patients resulting in full range of motion and pain relief.
Finally, hand-therapist, Elizabeth Kabel, DPT, was awarded scholarship funds for her contributions in documenting the treatment and outcomes of a professional motocross racer who underwent a novel, not previously reported, minimally invasive surgical technique. The patient was a 23-year-old professional motocross racer who presented with a six-week history of chronic right-sided forearm pain that was causing his hands to go numb and feel weak while riding his motorbike. The pain was so severe that he was unable to continue riding. He was treated unsuccessfully with conservative management before presenting for surgical intervention. The patient was diagnosed with chronic exertional compartment syndrome of the forearm (CECS). CECS of the forearm is a very painful and dangerous condition that motocross racers often refer to as “arm-pump.” CECS is traditionally treated with open compartment release requiring large incisions (nearly the entire length of the forearm) that can result in long recovery times and less than optimal aesthetic results. Dr. Cobb used an endoscopic technique to perform the surgery through a small incision. The patient’s symptoms resolved following surgery. He resumed riding at one week, competing at three weeks, and continues to ride competitively without symptoms. Images of the traditional surgical approach and the minimal scarring following technique used by Dr. Cobb are shown in Figures 1 and 2 respectively.

Quad Cities Orthopaedic Surgeons Detail Hidden Costs of Cubital Tunnel Syndrome

Dr. Tyson Cobb discuses how the costs associated with cubital tunnel syndrome affect employer and employee and explain the benefits of minimally invasive surgery.

Davenport, IA— Cubital tunnel syndrome is the second most common condition involving nerve compression of the arm after carpal tunnel syndrome, Dr Cobb. They recently explained how this prevalent malady can often cause healthcare costs to rise dramatically in a workplace environment for both the employer and the employee. In response to this concern, they recommend minimally invasive surgery to help reduce costs, eliminate personal discomfort, and increase workplace efficiency.

The practice explains that while there is currently no link between office work and cubital tunnel syndrome, there can be both direct and indirect costs for employers and employees when an individual suffers from this condition. HAND, April 2014) demonstrated that patients are able to get back to full duty 63 days sooner following endoscopic cubital tunnel. The orthopaedic surgeons further emphasize the benefits of this operation by indicating the rapid recovery period can mean a savings of up to $436 million annually for the United States. Healthcare costs in the nation can be staggering, they suggest, and this opportunity to reduce fees can amount to an economic windfall.
